Resumo: Medication-related osteonecrosis of the jaw (MRONJ) is a serious adverse event associated with different medications. A 69-year-old woman, non-smoker and non-alcohol consumer, using denosumab for 5 years, sought care complaining of jaw pain. On intraoral examination, a fistula with suppuration was noted in the alveolar ridge region on the right side, and on extraoral examination, a 2-cm rigid nodule with a reddish surface was noted, which had been present for 3 months. Cone Beam Computed Tomography revealed extensive hypodense changes in the bone trabeculation throughout the mandible, especially in the body and chin regions on the right side, and irregularity of the bone cortices. Due to the history of use of denosumab and the clinical and radiographic signs, she was diagnosed with advanced MRONJ. The patient did not want to undergo segmental resection and responded favorably to treatment with non-steroidal anti-inflammatory drugs, systemic antibiotics and photobiomodulation.
