Título: LATE DIAGNOSTIC OF GRANULOMATOSIS WITH POLYANGIITIS LEADING TO OROFACIAL MORBIDITY AND SYSTEMIC IMPACT

Resumo: A 44-year-old woman presented saddle-nose, facial pain, cacosmia, and generalized arthralgia. Her medical story revealed that this condition was empirically treated with continuous prednisone 40mg/day for 3 years and evolved to deformity of the fingers and wrist. Furthermore, the nasofibroscopy and computed tomography showed an alteration of the nasal architecture, a destruction of the upper respiratory tract followed by communication with the cervical spine, and a large palatal destruction. This condition was treated with antibiotics without success for six months. Therefore, the patient was referred for evaluation of this large necrotic palate perforation with antral communication and cervical pain. An incisional biopsy of the palate region was performed and showed vasculitis of small and medium vessels, compatible with a diagnosis of Granulomatosis with polyangiitis, which was confirmed by the positive C-ANCA serological test. Thus, was started the treatment with cyclophosphamide and the patient maintained clinical monitoring without disease progression.
