Resumo: Dermoid cyst consists of a cystic malformation of unusual development that can arise in any region of the body including the oral cavity, though occurrence is rare. They can be clinically confused with various close differentials including tumours, embryonic abnormalities and ranula. A 27-year-old female patient presented with a complaint of painless large swelling beneath the right side of the floor of the mouth. Intraoral examination demonstrated an exuberant movable elastic soft mass measuring approximately 40 mm in extension. The radiologic and tomographic assessment revealed a non-homogeneous, poor-defined lesion in the sublingual space without bone involvement. Under local anesthesia, surgical incision was implemented and the lesion was completely removed. The histopathological examination demonstrated a cavity filled with keratin, covered by keratinized stratified epithelium, whose capsule was constituted by fibrous connective tissue containing sebaceous glands, confirming the dermoid cyst diagnosis. Patient remains in follow-up without recurrence for three years.
