Resumo: A 19-year-old male patient presented for dental evaluation due to pain originating from the area around tooth 48. Upon clinical examination, it was noted that the third molar was impacted. A panoramic radiograph revealed a unilocular hypodense lesion with well-defined borders, without cortical expansion in the crown region of the affected tooth. Based on clinical and radiographic findings, the diagnostic hypothesis was of paradental cyst. Subsequently, tooth 48 was extracted along with the associated lesion. Histopathological analysis showed a thin fibrous capsule lined by uniform thickness stratified squamous epithelium, with a palisade basal layer and corrugated parakeratin on the surface. The definitive diagnosis was odontogenic keratocyst. The patient is currently being monitored regularly and shows no signs of recurrence.
