Resumo: Radiotherapy-induced oral mucositis is a debilitating acute oral complication of head and neck cancer treatment. The present case series shows three patients undergoing radiotherapy for head and neck tumors who developed grade 3 oral mucositis (WHO scale) within the first week of treatment, with very similar clinical presentation. In all cases, patients had a previous history of gastroesophageal reflux, with symptoms worsening during radiotherapy. Radiation dose was 65 - 70 Gy, with intensity-modulated radiotherapy (IMRT), associated with Paclitaxel (Patient 1), target-therapy (Patient 2) or exclusive radiotherapy (Patient 3), and they all received daily photobiomodulation to prevent oral mucositis. All patients had significant improvement in mucositis lesions after controlling gastroesophageal reflux. These clinical observations may motivate studies to evaluate the role of gastroesophageal reflux in the emergence of early severe oral mucositis in patients undergoing radiotherapy, so that preventive measures can be adopted if a cause-and-effect relationship is scientifically established.
