Resumo: A 70-year-old female patient presented with desquamative gingivitis and painful symptoms in the posterior region of the gingiva. The lesions appeared two months previously and in the subjective examination, the patient reported moments of stress at work. The bilateral posterior jugal mucosa was slightly rough and leukoplastic. Under the clinical hypothesis of lichen planus and mucous membrane pemphigoid, an incisional biopsy was performed on the gingiva of the right inferior molar. Microscopically, fragments of mucosa were observed with keratinized stratified squamous lining epithelium with areas of hydropic degeneration and rectification of the basal layer, completely detached from the connective tissue. Also, intense diffuse chronic inflammatory infiltrate, congested blood vessels, and areas of hemorrhage were observed. The final diagnosis was compatible with mucous membrane pemphigoid. The patient was referred to a rheumatologist and is now about a one-year follow-up, using corticosteroid mouthwashes during periods of exacerbation of the disease.
