Resumo: Adenoid ameloblastoma (AdAM) presents variable diagnostic criteria and clinical behaviours. We report two cases of AdAM in male patients in their eighth decade of life. In case I, a patient previously diagnosed with ameloblastoma presented painless facial asymmetry and a 3.5 cm nodule in the left maxillary sulcus. In case II, the patient presented pain and teeth mobility without swelling or facial asymmetry. Computed tomography showed hyperdense areas in facial sinuses, bone thinning, and erosion of maxillary bone in case I, whereas case II showed a hypodense lesion with root destruction of involved mandibular posterior teeth, extending to ramus. Exploratory aspiration yielded negative results and was followed by a biopsy. Histopathology revealed an ameloblastoma-like component organized in cords and islands, along with duct-like structures, cribriform architecture and dentinoid material, leading to the diagnosis of AdAM. Surgical resection was performed, with patients remaining under observation and no recurrence detected.
