Resumo: Hybrid lesions containing a component of giant cell granuloma with peripheral ossifying fibroma in peripheral location is extremely rare. A 69-year-old woman was referred to the Department of Dentistry for evaluation of a lesion on the left lower alveolar ridge that appeared over 30 days ago and has not regressed and it was not related to local trauma. On the physical exam, the presence of a 1cm painfull well-defined reddish pedunculated nodular lesion in the mandible, with irregular surface and fibroelastic consistency. An excisional biopsy of the lesion followed by histopathologic examination of the specimen revealed distinctive features of peripheral giant cell granuloma and peripheral ossifying fibroma comprising proliferation of oval to spindle shaped mononucleated mesenchymal cells, interspersed with multinucleated giant cells intermixed by extensive deposition of metaplastic trabecular bone. The final diagnosis was peripheral giant cell granuloma with hybrid peripheral ossifying fibroma. The patient remains under close follow-up.
