Resumo: The nasopharyngeal angiofibroma is a rare, locally aggressive, highly vascular, benign tumor, characterized by mostly occurring in young males. Association with other pathologies is rare. Herein, we report a 23-year-old male showing an increased volume in the middle third of the left face, with one year of evolution. The imaging exam showed a radiolucent area with a radiopaque halo, with approximately 5 centimeters, on the maxilla. After an incisional biopsy, the histopathological report identified an odontogenic keratocyst. However, the marsupialization was unsuccessful, and an excisional biopsy was performed. Histopathological analysis showed a high proliferation of blood vessels and myoid-like cells, covered by fibrous connective tissue, in addition to an odontogenic cyst in the peripheral area. Immunohistochemistry positivity for vimentin, CD34, α-SMA and Ki67 guides the diagnosis to mesenchymal benign neoplasia compatible with nasopharyngeal angiofibroma associated with odontogenic keratocyst. The patient has been monitored for 6 months to verify possible recurrences.
