Resumo: Myofibromas are benign myofibroblastic tumors frequently affecting male infants and children in head and neck region. The typical microscopy is a well-circumscribed neoplasm with biphasic architectural pattern flanked by myoid nodules. Relevantly, a small subset (about 10%) of myofibromas displays atypical features, which can suggest low-grade sarcoma. Case 1: 17-year-old male patient presented with a rapidly growing lesion on the right side of the mandible 1-month ago. After biopsy, microscopy revealed typical features of myofibroma. Case 2: 12-year-old girl presented an extensive and destructive osteolytic lesion, with tooth displacement, on the left maxilla, several weeks ago. After biopsy, microscopy showed hypercellularity, mitoses, absent myoid nodules, and infiltrative growth pattern, suspicious for spindle cell malignancy. However, the strict clinicopathological correlation favored atypical myofibroma. By immuhistochemistry, α-SMA, calponin and Ki-67 (case 1, <1%; case 2, 6%) were positive. In both cases, after 1-year follow-up, there were no alterations.
