Resumo: Multifocal proliferative leukoplakia (MPL) is an oral potentially malignant disorder (OPMD) characterized by a high malignant transformation rate. Its clinical presentation varies widely, with different patterns and extents of involvement, with greater invasiveness observed when it affects the gingival region. Here, we present the case of a 65-year-old non-smoking, non-drinking man with multiple white plaques affecting the buccal and lingual gingiva around teeth 46 and 47, extending into the furcal space and causing tooth mobility. Biopsies revealed oral epithelial dysplasia, while cone beam tomography showed invasion of the periodontal space. Initial treatment included diode laser ablation and extraction of the affected tooth, but recurrence occurred after 2 months, necessitating resection with wider margins. This case underscores the invasive nature of MPL when involving gingival structures and highlights the importance of rigorous and lifelong monitoring for such lesions.
