Resumo: Juvenile ossifying fibroma (JOF) is a rapidly growing benign fibro-osseous neoplasm. Histopathologically, JOF can be classified into trabecular and psammomatoid subtypes. The gold standard treatment for JOF is radical resection with safe margins, which causes facial defects that require reconstruction to re-establish aesthetics and function. Our study presented a series of five JOFs treated with conservative resection and orbital reconstruction with iliac crest bone graft. After the surgical approach, maintenance of the vertical and horizontal position of the eyeball was observed, with no severe development of enophthalmos, diplopia, or ophthalmoplegia. Facial symmetry, mouth opening, nasal perfusion, and visual acuity were considered satisfactory. The patients have been followed up for a minimum of one year and a maximum of twenty-five years, with no signs of recurrence. Therefore, this study showed that the iliac crest graft is a satisfactory option for maxillo-orbital reconstruction after FOJ removal.
