Título: ACINIC CELL CARCINOMA WITH HIGH-GRADE TRANSFORMATION: REPORT OF A RARE CASE

Resumo: A 60-year-old female patient came to our service due to a six-month-old lesion in the right mandibular angle with painful symptoms. Computed tomography revealed a nodular image in the right parotid gland, in contact with the styloid process and right jugular vein, and cervical lymph node enlargement at level IV. Ultrasound-guided aspiration showed a hypoechoic intraparotid nodule with cytological characteristics indicative of a basaloid/follicular neoplasm. The patient underwent parotidectomy with total right cervical dissection. Histologically, the tumor presented as neoplastic cells with serous acinar cell morphology, with basophilic cytoplasmic zymogen granules arranged in a solid, microcystic, and follicular pattern, and an area with increased nuclear pleomorphism, coarse chromatin, necrosis, increased mitotic activity and Ki-67 index. After microscopic and immunohistochemical analysis, the diagnosis of acinar cell carcinoma with high-grade transformation was confirmed, with metastasis to the intraparotid lymph node and compromised margins. The patient is currently undergoing adjuvant radiotherapy.
