Título: ATYPICAL AND PSEUDOMALIGNANT FEATURES OF SALIVARY PLEOMORPHIC ADENOMA: A CLINICOPATHOLOGIC STUDY

Resumo: Objective: Pleomorphic adenoma (PA) exhibits a diverse histological spectrum characterized by epithelial and mesenchymal patterns, secretions, and cellular bipopulation. However, a subset of PAs shows microscopic features suggesting malignancy, leading to their classification as "atypical PA". Study Design: This study conducted a thorough retrospective analysis of 84 cases from 1960 to 2023, aiming to categorize PAs with malignancy-indicative features and prevent misdiagnosis. Results: Cases were categorized into mimickers of invasion (MI), mimickers of malignant cytomorphology (MMC), and mimickers of defined malignant tumors (MMT). In MI cases (19%), satellite nodules predominated (56.2%), with additional findings including capsular invasion, irregular growth, pseudopodia, and vascular permeation. MMC features were noted in 89.2% of cases, comprising increased cellularity, cellular atypia, increased mitotic activity, oncocytic metaplasia, and necrosis. Additionally, 25% of PAs were classified as MMT, with myoepithelial carcinoma, polymorphous adenocarcinoma, and adenoid cystic carcinoma being the most common subtypes exhibiting overlapping features. Conclusion: Despite PAs diagnosis being generally straightforward, cases with these features may be mistakenly interpreted as carcinoma. The shared morphocytological features underscore the complexity of accurate diagnosis, emphasizing the need for meticulous examination and a comprehensive assessment, incorporating morphological, molecular, and immunohistochemical analyses to differentiate between benign and malignant salivary gland tumors.
