Resumo: Verrucous carcinoma is a rare low-grade subtype of squamous cell carcinoma with an uncertain etiology. A 62-year-old male complained of oral pain, weight loss, and progressive tongue lesions for over 8 months. Intraoral examination revealed bilateral white plaques in the buccal mucosa and a 6 cm exophytic and papillomatous mass on the tongue. A head and neck CT revealed hyperdense area on the right side of the tongue without evidence of lymph node involvement or laryngeal lesions. Incisional biopsy and histological examination suggested a verrucous carcinoma. The patient underwent conservative partial glossectomy using an electric scalpel followed by second-intention healing. Photobiomodulation therapy was administered to relieve the pain and enhance wound healing. The treatment plan included regular 3-month follow-up consultations with the oral medicine and head and neck surgery teams. After 6-months, no recurrence was observed, and the patient reported improved symptoms with occasional tongue numbness.
