Resumo: Objective: This retrospective study aims to describe and compare the radiographic features of plasmablastic lymphomas (PBL) and multiple myeloma/plasmacytoma (MM) of the jaws in panoramic radiographs, looking for diagnostic tools for the differentiation of these neoplasms. Study design: A convenience sample of 20 panoramic radiographs from multiple centres of patients with PBL and MM were analysed. The diagnose was reviewed and the radiographic images obtained were standardized for evaluation. A table containing the clinical data and radiological features (radiodensity, borders, locularity, location, dental alterations and effect on anatomic structures) was analysed. Results: Most of PBL lesions on panoramic radiographs were unilocular with poorly defined borders, while lesions with well-defined borders and multilocular aspect were more frequent in MM. Although the punched out aspect is characteristic of MM, it was visualized in one PBL case. The PBL osteolytic lesions on panoramic images are restrict to two quadrants and MM have a more generalized aspect, usually affecting entire mandible. Conclusion: When suspecting of neoplasms of plasma cells or with plasmablastic differentiation, a panoramic radiographic image with unilocular aspect and poorly defined borders favours the characteristics of PBL. The punched out aspect is more frequent in MM, though it is not pathognomonic.
