Resumo: Gardner syndrome (GS; OMIM #175100) is a severe variant of familial adenomatous polyposis characterized by adenomas in the colon and rectum coupled with extracolonic manifestations including osteomas and soft tissue tumors. A 30-year-old female presented for diagnosis of a radiographically hyperdense lesion in the posterior left maxilla, where a supernumerary tooth was also identified. She had previously undergone ileostomy due to intestinal polyps. Her father exhibited similar features and had a history of intestinal cancer. A biopsy in the left maxilla was performed and revealed a yellowish-white mass with small hard fragments. Histological analysis revealed a minimally cellularized hard tissue, in some areas resembling dental cementum. Islands and cords of apparent odontogenic epithelium were identified in limited dense fibrous connective tissue, confirming the diagnosis of odontoma. The co-occurrence of intestinal polyps and odontoma led to a diagnosis of GS. The patient awaits surgery for lesion removal and remains under supervision.
