Resumo: Central Ossifying Fibroma is an uncommon benign neoplasm of fibro-osseous origin that affects the bones of the face, diagnosed by clinical, radiographic and histopathological examinations. The lesion is often asymptomatic, progressing to swelling and noticeable deformities. The aim of this work is to report a case of Central Ossifying Fibroma in the right mandibular body and ramus, identified by a panoramic radiography for the purpose of dental treatment, characterized by an osteolytic lesion in a 41-year-old melanodermic asymptomatic patient, despite its large dimensions, confirmed by incisional biopsy, consisting of two fragments of irregular white and brown soft tissue and four fragments of brown hard tissue. A Multidetector Computed Tomography scan was taken to locate the limits and extent of the osteolytic lesion, without cortical disruption, with hyperdense areas inside, measuring 4.1 x 2.6 cm, displacing the mandibular canal. The patient is currently awaiting enucleation and curettage surgery scheduled by SUS.
