Resumo: The present case is a report of a 58-year-old male patient with a hard, asymptomatic swelling in the palate region. During the anamnesis, he reported not having noticed the appearance of swelling in the palate. Intraoral examination showed an elongated nodule of hard consistency on the right hard palate, covered by intact mucosa of normal color, measuring approximately 25x10 mm. Computed tomography showed a heterogeneous, multilocular, circumscribed hyperdense area, with small hypodense areas, presenting a pedicle in the palatal bone cortex between teeth 14 and 15, in addition to bone union with the posterior region of the ipsilateral hard palate. The main hypothesis was peripheral osteoma. An excisional biopsy was performed and microscopic analysis showed fragments of lamellar bone with osteoid deposition on the periphery of the trabeculae, in addition to medullary spaces. The diagnosis of peripheral cancellous osteoma was established. Clinical-radiographic follow-up revealed no signs of recurrence after 12 months
