Resumo: A 20-year-old male patient presented to the stomatology service with complaints of dysphonia and dysphagia. Upon clinical examination, a vegetative, asymptomatic lesion was observed located on the lateral border of the tongue on the right side. An incisional biopsy was performed, and the histopathological examination revealed a benign mesenchymal neoplastic lesion of probable muscular origin, with predominantly epithelioid cells with bizarre nuclei. An immunohistochemical panel was requested, which showed positivity for smooth muscle actin, desmin, vimentin, and Ki-67 proteins. Combining the clinical, histopathological, and immunohistochemical information confirmed the muscular origin of the lesion, a Leiomyoma. Partial glossectomy with right lateral margin was performed by the head and neck service of an oncological hospital. The patient has been followed up by the hospitals dental service for 8 months without signs of recurrence.
