Resumo: A 58-year-old male was referred for evaluation of a painless lesion in anterior maxilla following endodontic treatment, presenting with mobility of the maxillary incisors. His medical history was no significant. A histopathological analysis of an incisional biopsy revealed a proliferation of small, round, blue cells and areas of large cells, initially indicating lymphoma. To differentiate between diffuse large B-cell lymphoma (DLBCL), Burkitt lymphoma, and plasmablastic lymphoma, a deep learning model leveraging the Xception architecture was employed. This model, trained on 41,227 image patches from whole slide images of 30 patients, achieved a diagnostic accuracy of 95.24%, an F1-score of 0.95, and a ROC AUC of 96.39%. Immunohistochemistry further confirmed DLBCL, with positive results for LCA, CD20, PAX5, CD10, BCL-2, BCL-6, and a high KI-67 index (80%). This case highlights the transformative potential of integrating artificial intelligence with traditional histopathological techniques, significantly improving diagnostic accuracy and efficiency.
