Resumo: A 50-year-old female arrived at the dental clinic with a painful nodule on the right oral mucosa, which appeared a month ago. A mobile submucosal nodule was noted during physical examination, with a smooth surface, similar in color to the adjacent mucosa, soft consistency, measuring approximately one centimeter. The patient reported having undergone an injectable aesthetic procedure with a calcium hydroxyapatite collagen biostimulator, close to the region, about a year ago. After ultrasound compatible with lipoma and under the clinical diagnostic hypothesis of foreign body reaction, an excisional biopsy was performed. Microscopically, adipose tissue fragments were observed amidst areas of connective tissue with granulomatous inflammation. Refringent material with a crystalloid appearance and amorphous basophilic material associated with multinucleated giant cells were also visible, with rare well-formed granulomas. The final diagnosis was a foreign body reaction to exogenous material. She has been followed for over a year with no recurrence.
