Resumo: A 35-year-old female patient presented with a firm, ulcerated sessile nodule measuring approximately 25x15mm on the left buccal mucosa, extending to the mandibular fornix. The lesion had been noticed by the patient for ten years, and a biopsy performed two years ago initially suggested mechanical trauma as the cause. However, differential diagnoses included squamous cell carcinoma and malignant mesenchymal or glandular neoplasms. An incisional biopsy revealed a salivary gland neoplasm characterized by small myoepithelial and ductal cells with basophilic nuclei arranged in a cribriform pattern, ultimately diagnosed as adenoid cystic carcinoma. Surgical resection of the tumor was undertaken, but the margins were positive for neoplasia, prompting a subsequent surgical intervention after five months. Radiotherapy was not indicated. The patient has since been under regular clinical observation and has shown no signs of recurrence in the twelve months following treatment.
