Resumo: Female, 56-years-old, had a medical history of oral squamous cell carcinoma in the left side of the tongue, treated with glossectomy and radiotherapy (RTx). Four months after the RTx, the patient developed bilateral osteonecrosis (ORN) in the jaw. During the preparations for ORN´s mandibular surgery, ORNs were treated with ozonetherapy and hyperbaric oxygenation, plus to the PENTOCLO protocol. The patient presented another progressive and painful tumor on the right side of the mandíbula. The biopsy shows a high-grade pleomorphic sarcoma. A panoramic x-ray and tomography were performed, in which a destructive lesion was observed in the body and right mandibular branch with infiltration into adjacent tissues. The sarcoma and ORN of the right side were treated by hemimandibulectomy without free margins. The histopathology was conclusive of osteoblastic osteosarcoma, which was probably secondary to radiotherapy. Nowadays, two months after surgery, the patient has functional and aesthetic sequelae and remains under follow-up.
