Resumo: Central Odontogenic Fibroma (COF) is a rare benign mesenchymal odontogenic tumor of the jaws. In most cases, it is a slow-growing asymptomatic lesion. In this case report, a 30-year-old female patient presents a well-defined multilocular radiolucent lesion involving the body and ramus of the right mandible, around the second pre-molar and extending until the third molar, with displacement of this and discrete resorptions of the root apices of the tooth 46. An incisional biopsy was performed, with an inconclusive result. In the second surgical approach, since the lesion presented an easy cleavage plane, the lesion was totally removed. The microscopic examination revealed a mesenchymal neoplasm, composed by fibrous connective tissue, presenting small nests and cords of non-proliferative odontogenic epithelium, characteristic of COF. A reconstruction plate was used to prevent pathological fracture. The patient is under control with no signs of recurrence and awaiting rehabilitation with bone graft, implants and prosthesis.
