Resumo: Orthokeratinized odontogenic cyst is a rare type of developmental odontogenic cyst that was previously considered a variant of odontogenic keratocyst. The purpose of this report is to describe a case of an 18-year-old female who was diagnosed with an orthokeratinized odontogenic cyst. The patient presented with a mandibular swelling for about three months. Radiographic examination revealed a well-defined, unilocular, radiolucent lesion with a corticated margin in the posterior body of the mandible. An incisional biopsy was performed under the clinical suspicion of an odontogenic keratocyst. Microscopic examination revealed thin, regular epithelium with 2-3 cell layers and a prominent granular cell layer with orthokeratinization. Following a final diagnosis of an orthokeratinized odontogenic cyst, an excisional surgery was performed. The patient underwent a 6-month follow-up, and a postoperative panoramic radiograph showed new bone growth in the area of the cystic defect with no signs of recurrence.
