Resumo: Introduction: Face destruction forces us to create new reconstruction techniques. Objetivo: To describe the technique of bone fitting with the objective of improving the stability between the microvascular fibular flap (MMF). Method: The end portion of the MMF is cut at 50% of its thickness, creating a bone contact area of approximately 1cm at its extremity, which is positioned on the external or buccal face of the mandible (Preserving dental roots in dentate patients). The flap is fixed with titanium miniplates and screws. Case Report: Female patient, aged 17 years, with ameloblastoma involving the anterior region and body of the mandible up to the bilateral first molar. She underwent immediate resection and reconstruction with MFF. After 20 months, stability of the reconstruction was noted, with maintenance of facial function and aesthetics. Conclusion: With this technique two contact surfaces between the segments are obtained, facilitating the consolidation.
