Resumo: Jaw osteosarcoma is infrequent posing challenges in timely diagnosis and potentially delaying treatment initiation. A 46-year-old male presented left mandibular swelling persisting for 6 months. During the initial consultation, the patient reported undergoing an excisional biopsy 3 months prior, presumptively diagnosed as pyogenic granuloma. The sample obtained was not submitted for histopathological analysis, and the lesion continued growing. Clinical examination revealed a tumor-like lesion measuring approximately 4x4cm extending vestibulolingually from teeth 34, 35, and 36. It appeared hardened, with sessile implantation, lobular surface, and color similar to the adjacent mucosa with ulcerated areas. Left cervical lymphonodomegaly was observed. Imaging revealed periodontal ligament widening, and cortical bone expansion. A new biopsy was performed reporting an osteoblastic osteosarcoma. The patient underwent partial mandibulectomy, unilateral lymphadenectomy, chemotherapy, and radiotherapy. Delay in the diagnosis of jaw osteosarcoma complicates clinical management, necessitating extensive surgery and aggressive treatment. Histological analysis is mandatory for accurate diagnosis.
