Título: SURVIVAL OF PATIENTS WITH ORAL CANCER IN URBAN AND RURAL AREAS: A SYSTEMATIC REVIEW AND META-ANALYSIS

Resumo: Objective: Aimed to compare oral cancer patients survival in urban and rural regions. Study design: A systematic search was performed across PubMed, Embase, Web of Science, Lilacs, Scopus, and ProQuest databases. Eligibility criteria included observational studies, without language or publication date restrictions. Meta-analysis employed Mantel-Haenszel tests for dichotomous outcomes, presenting odds ratios with 95% confidence intervals. Statistical significance was established at p > 0.05. The Random Effects model was used for high heterogeneity values (p > 0.10). Results: 467 studies were found, 26 analyzed in full text, 5 included in qualitative synthesis and 4 in meta-analysis. A total of 48,951 patients (83% urban/17% rural) with a mean age of 60.5 years. The average observation period was 20.7 years, with an 83-month follow-up. 3-year survival rate was 69% for rural and 73% for urban areas, while 5-year survival rate was 63% for urban and 67% for rural areas. Meta-analysis revealed that there is no difference in 3-year survival (p= 0.42, MH: 1.16, 95% CI: 0.81- 1.64) and 5-year survival (p= 0 .19, MH: 0.66, 95% CI:0.35- 1.23) of patients in urban and rural areas. Conclusion: The data suggests no evidence supporting higher survival rates for urban patients compared to rural ones.
