Título: ANALYSIS OF GRADING SYSTEMS IN ORAL SQUAMOUS CELL CARCINOMA REGARDING SURVIVAL

Resumo: Objective: apply three grading systems (Malignancy Grading of the Deep Invasive Margins, World Health Organization grading system, and Histologic Risk Assessment) to oral squamous cell carcinomas (OSCC) and evaluate each system based on clinicopathologic parameters and patient survival. Study design: Sixty paraffin-blocks from patients diagnosed with primary oral SCC at the Cancer Center were assessed. All hematoxylin and eosin-stained slides were reviewed to confirm the diagnosis and determine histopathologic grading. Clinicopathologic data were retrieved from medical records and tumor registries. S-100 immunohistochemistry analysis was also conducted. Statistical analysis included Fisher's exact test or the chi-square test, the Kaplan-Meier method, and the log-rank test. Results: The World Health Organization grading system showed a statistically significant association with tumor size (p=0.006). Malignancy Grading of the Deep Invasive Margins was correlated with age (p= 0.003), and Histologic Risk Assessment was associated with pain (p = 0.0006). Significant differences in cancer-specific survival were observed for Histologic Risk Assessment (p= 0.04), lymph node status (p < 0.001) and perineural invasion (p=0.001). Simultaneous surgical, radio, and chemotherapy treatment was associated to better survival (p= 0.001). Conclusion: Among the grading systems evaluated, Histologic Risk Assessment demonstrated the most accurate predictions for survival in OSCC.Funding: FAPEMIG (APQ 02833/18)
