Título: COMPLEXITY ON SJOGREN'S SYNDROME DIAGNOSIS: A CASE REPORT

Resumo: A female patient, 31 years old, attended to the dental office complaining of painful swelling on parotid and submandibular region that was present for years. After several appointments with different health professionals, she reported some pain relief after sporadic anti-inflammatory therapy over the years, but no diagnosis. Besides the symmetric bilateral swelling, skin and mucosal dryness were evident as well as a reddish face. Blood sample for autoimmune conditions and infectious diseases (hepatitis, HIV, syphilis) were performed along with biopsy of lower lip minor salivary glands. Blood sample was negative for infectious diseases, reagent for FAN nuclear antibodies (1/640), rheumatoid factor (120 IU/mL), and Anti-SSA/RO (227 U/mL). Microscopy evidenced chronic sialadenitis spread across gland lobes, especially around ducts. There were no germinative center, and the index for lymphocytic focus was 2,59 focus/4 mm2, compatible with Sjogren's syndrome. Patient was referred to a rheumatologist and has been followed for dental care.
