Resumo: Sarcomas, originating from mesenchymal cells, constitute a rare and heterogeneous group of neoplasms, representing about 1% of all malignant oral cavity tumors. An 8-year-old female patient, previously diagnosed with a mucocele and having the lesion removed elsewhere, presented to the dental service with a lesion near the retromolar trigone. It manifested as a pedunculated tumor with necrotic center on the right mandibular side. Following radiographic examination, an excisional biopsy was performed. Histopathological examination revealed a proliferation of relatively monomorphic oval shaped cells with clear to pale, eosinophilic cytoplasm, delicate speckled chromatin, and small variably conspicuous nucleoli, with frequent mitoses, compatible with soft tissue sarcoma. The patient was referred to the oncology department where immunohistochemical analysis showed positivity for MDM2, CDK4, STAT6, and negativity for SOX-10, DESMIN, SMA, CK (40, 48, 50, 50, 6 kDa). The patient underwent surgery and radiotherapy and is currently under treatment surveillance.
