Resumo: Central odontogenic fibroma (COF) is a benign tumor originating from dental ectomesenchymal tissue, equally affecting the maxilla and mandible, with a female predilection. A 15-year-old Caucasian female presented with delayed eruption of tooth 23. On panoramic imaging examination, a well-circumscribed and corticalized radiolucent lesion was noted, associated with the crown of tooth 23. A cone beam computed tomography was requested in which tooth 23 was seen to be impacted, torn apart and in contact with the left maxillary sinus, with a hypodense and expansive lesion associated with its crown, causing palatal cortex resorption. Therefore, an excisional biopsy was performed and histopathological analysis revealed loosely organized connective tissue with moderately cellularized ovoid, fusiform, and occasionally star-shaped cells. The tissue had a fibromyxoid appearance resembling dental papilla, along with small circular islands and scattered strands of inactive odontogenic epithelium observed in focal areas, confirming COF. Monitoring by a specialized team was recommended.
