Resumo: Oncocytic hyperplasia refers to a metaplastic transformation occurring in ductal and acinar cells, typically observed in major or minor salivary glands. It is rarely reported and presents an uncertain etiopathogenic process, potentially associated with inflammatory fibrous hyperplasia (IFH). We present the case of a 66-year-old Caucasian woman, a non-smoker and non-alcohol consumer, who presented with longstanding multiple nodular lesions along the mucosa of the maxillary alveolar ridge. Clinical examination led to a diagnosis of IFH, subsequently confirmed via excisional biopsies. Histopathological examination revealed typical IFH features alongside marked cell hyperplasia with oncocytic metaplasia in ductal structures, exhibiting irregular arrangements in both single and double-layered structures with cystic dilatation. Focal hyperplasia and thickening were noted in some excretory ducts. The patient is being followed up. This case underscores the co-occurrence of oncocytic hyperplasia with IFH, emphasizing pathologists need to recognize such features to avoid misdiagnosis and unnecessary investigations.
