Resumo: Leiomyosarcoma (LMS) is a malignant neoplasm that affects smooth muscle tissue and its occurrence in the mouth is rare. Extensive surgical resection is indicated due to the high recurrence rates, which can make future oral rehabilitation difficult. We report a case of a 35-year-old female patient who underwent a biopsy of an expansive lesion in the mandible diagnosed with LMS. The patient was referred to the São Paulo State Cancer Institute where she underwent neoadjuvant chemotherapy followed by segmental mandibulectomy, reconstruction with a microvascularized fibula flap and adjuvant radiotherapy. After a year, the rehabilitation process began with thinning of the flap, installation of integrated bone implants and rehabilitation with a protocol prosthesis. The patient has been followed up regularly, showing good adaptation to the prosthesis and improvement of her functions. This case emphasizes the need for multi-professional care in order to improve the patients quality of life.
