Resumo: A 70-year-old white male presented complaining of "sore mouth and eating difficulty" with 4 years' evolution time. He had a previous history of penicillin allergy, smoking, and alcoholism. Physical examination revealed local cervical lymphadenopathy and a 5 cm ulcerated lesion with irregular edges on the right posterior mandibular alveolar ridge. Cone-beam computed tomography (CBCT) showed a hypodense image with irregular margins in the affected area, with a "moth-eaten" feature. The presumptive diagnosis was squamous cell carcinoma (SCC); incisional biopsy was performed and microscopy revealed islands of neoplastic epithelial cells with slight pleomorphism, hyperchromatism, atypical mitotic figures, keratin pearls, invading the connective tissue, with an intense mononuclear inflammatory infiltrate. The diagnosis established was Squamous cell carcinoma. The patient was referred to an oncologist. The epidemiological profile of individuals with oral SCC hinders early diagnosis, compromising patient's survival. Tracking and prevention actions in targeted populations should be the stomatologist's focus.
