Resumo: A 55-year-old female patient presented with an ulcerated lesion with a granulomatous appearance in the palate extending from the cervical region of the teeth, also affecting the vestibular surface. The lesion measured 2 cm and was asymptomatic. Previous medical history revealed a recent weight loss. The first diagnostic impression was paracoccidioidomycosis or another granulomatous lesion. Three incisional biopsies revealed chronic granulomatous infiltrate. Grocott-Gomori and PAS were negative for fungus infection. Acid-fast bacilli bacteria were not detected in Ziehl-Neelsen. Following the investigation, a chest X-ray revealed radiopaque lesions related to a fibro cicatricial process, suggestive of bronchiectasis. Tuberculosis was then suspected and the Mantoux test and sputum examination confirmed the diagnosis (13 mm diameter induration after tuberculin application). The patient was referred for treatment and reported remission of the lesions after the beginning of the medication. In conclusion, oral tuberculosis diagnosis is complex and requires correlation with adjuvant tests.
