Título: A SURPRISING FINDING IN A PATIENT WITH A CLASSIC PROFILE FOR ORAL SQUAMOUS CELL CARCINOMA: CASE REPORT

Resumo: A 45-year-old woman, who was a smoker and an alcoholic, sought medical attention reporting a sensation of having a throat full of pus. Upon examination, white mucous plaques were observed on the lower labial mucosa. An incisional biopsy was performed. Within 7 days, an asymptomatic ulcer emerged on the right side of her tongue, exhibiting rapid growth. A second biopsy was conducted. Based on clinical and anamnestic data the diagnostic considerations leaned towards syphilis or squamous cell carcinoma. In histological analysis, the mucosa was partially covered by atrophic squamous epithelium. An intense chronic inflammatory process with a predominance of plasma cells arranged in a perivascular pattern was present in the connective tissue. Immunohistochemical tested positive for anti-treponema pallidum antibodies. Supported by clinical, histopathological, and immunohistochemical evidence, a diagnosis of secondary syphilis was made. Prompt referral to a specialized hospital assured successful treatment without recurrences.
