Resumo: Proliferative verrucous leukoplakia (PVL) is a multifocal, progressive lesion, with aggressive biological behavior due to its high rate of malignant transformation, unknown etiology, resistant to therapy and frequent recurrences. An 84-year-old woman diagnosed with PVL for 8 years, complaint of pain on the oral mucosa (VAS=7) and inability to wear dentures. Lesions extended throughout buccal mucosa, palate, mouth floor, tongue and gums, extensive, rough plaques with well-defined edges, firm consistency and ulcerations. Three areas were biopsied, microscopic features showed mucosa with keratinized stratified squamous epithelium, areas of intense polymorphonuclear exocytosis. Epithelial dysplasia, hyperchromatism, pleomorphism, rare dyskeratosis, increased number of mitoses and loss of nucleus/cytoplasm ratio. Epithelial ridges were bulging. Underlying, there is fibrous connective tissue with inflammatory cells. On the surface, foci of Munros microabscesses. The diagnosis of PVL was given based on association of microscopic findings and clinical history of recurrent leukoplakic lesions. The patient remains under clinical follow-up.
