Resumo: Cemento-osseous dysplasia (COD) is a benign fibro-osseous lesion, characterized by the replacement of normal trabecular bone with osseous tissue and dense acellular cementum in a fibrous stroma. The condition does not require treatment except in the presence of symptoms, which normally occurs associated with Osteomyelitis. We report two cases of infected/osteomyelitis florid COD. Radiographically, both infected COD areas presented mixed density. Computadorized tomography exhibited no communication with oral cavity in the other regions. Case #1: A 52-year-old female pheoderm patient wearing double total prothesis reported spontaneous and intense pain for 2 weeks in the right posterior mandible. Case #2: A 52-year-old female melanoderm patient reported pain and purulent exsudate for a month. Symptoms initiated post the lower left premolar extraction. Curettage was performed and histopathology showed mineralized mass similar to cementum in both cases and bacterial colony, in case 2. Infected COD is uncommon and correct diagnosis provides appropriate treatment.
