Resumo: Mucocele of the paranasal sinuses is a benign, slow-growing, expansive lesion. Usually associated with painless bulging of the cheek, alveolar ridge expansion and orbital expansion. Caused by ostium ventilation blocking or endodontically treated maxillary posterior teeth. A 50-year-old man, with complaint of bulging of the cheek, after endodontic treatment of first maxillary molar. CBCT showed a unilocular, expansile lesion in the right maxillary sinus. Bony expansion in the lateral, medial, orbital, and downward directions with spherical shape, severely extended upwards, eroding the orbital floor. Upon aspiration, a red and visquous liquid. Classic Caldwell-Luc surgery and marsupialization were performed. Microscopic features of sinus mucosa, ciliated pseudostratified respiratory epithelium with mucous cells, intense mononuclear inflammatory infiltrate, cholesterol crystals, inflammatory multinucleated giant cells and fibroblasts, irregular brownish material between the cholesterol crystals, suggestive of extravasated endodontic material. Diagnosis of maxillary sinus mucocele was given. The patient remains under clinical and radiographic follow-up.
