Resumo: Carcinoma in situ is a dysplasia that affects all layers of the epithelium. Meanwhile, Human Papillomavirus (HPV), a sexually transmitted infection that increases the risk of cervical and oropharyngeal cancer, primarily subtypes 16 and 18. This report describes the case of a 39-year-old woman, immunocompromised due to Systemic Lupus Erythematosus and with a previous diagnosis of HPV in the cervix. The patient presented with an exophytic, whitish, sessile lesion with well-defined borders and a crusty surface, measuring 4x3 mm, on the lower lip. Excisional biopsy revealed cells with hyperchromatism, poor nuclei, and loss of stratification in all layers, as well as suggestive areas of viral inclusion, leading to a diagnosis of Carcinoma in situ. The margins of the lesion were expanded, and the patient was referred to a gynecologist and head and neck specialist. After 12 months of follow-up, the clinical presentation remains satisfactory.
