Resumo: Objective: To analyze the therapeutic results of a group of patients diagnosed with Ackermans verrucous carcinoma (VC) with and without associated potentially oral malignancy disorders (TOPM) treated with methotrexate (MTX) as first line treatment.

Materials and Methods: We designed a retrospective cohort study of 19 patients diagnosed with VC from 2015-2022 and treated with MTX. Inclusion criteria were tumor size equal to or greater than 1.5 cm in diameter, considering the possibility of a single lesion or multiple tumor lesions, and without previous treatment.

Results: The clinical response was the total reduction of lesions in 10 patients, greater than 50 % in 7 and less than 50 % in 2.

Conclusión: In this retrospective study, MTX showed promise as a first-line treatment in patients with CV. Healthcare users benefited from organ preservation through less extensive onco-stomatologic procedures. This led to an improvement in quality of life. Healthcare resources were optimized because the treatment was less expensive and allowed a greater number of users to be treated at the same time.
