Resumo: A 26-year-old woman was referred to a hospital dentistry service presenting a painful volume increase in the ventral surface of the tongue with one month of evolution. Upon physical examination, a bluish swelling, measuring 2,0 x 1,5 cm with a smooth surface was observed on the anterior right ventral surface of the tongue near the lingual frenulum. The patient reported a previous increase in the submental volume not observed upon extraoral examination. The clinical diagnosis of a Blandin-Nuhn cyst was established, and a marsupialization was performed due to the patients discomfort. A few days after the procedure, a new swelling was observed, but the patient reported less discomfort than the initial evaluation. For effective treatment, the complete removal of the lesion and the involved salivary gland is necessary. Therefore, it was decided to refer the patient for surgical excision at hospital-level care under general anesthesia.
