Resumo: A 73-year-old woman was referred to our service complaining of severe mandibular pain. She had a history of uncontrolled diabetes and recent osteoporosis, using Sodium Alendronate for approximately 12 months. After this period, she developed peri-implantitis and had an implant loss in the region, with several unsuccessful dental treatment attempts for a year. Through a detailed clinical examination and computed tomography, the diagnosis was stage 3 medication-related osteonecrosis of the jaws (MRONJ). Treatment plan involved controlling the infection, with subsequent surgery, with the combination of complementary therapies. Infection control involved antibiotic therapy, chlorhexidine digluconate rinses and antimicrobial photodynamic therapy (aPDT) sessions. The surgery was guided by optical fluorescence, with concomitant use of aPDT, L-PRF and photobiomodulation (PBM), with complete resolution of MRONJ. The relevance of this case lies in the importance of controlling risk factors in patients using osteolysis inhibitors to minimize the risk of MRONJ.
