Resumo: A wide range of white lesions can affect the oral mucosa, potentially indicating benign or malignant conditions. Therefore, obtaining clinical information associated with a representative biopsy is essential for an accurate diagnosis. This report aims to describe the case of a 64-year-old female who presented with a 3 mm white plaque with irregular borders and surface located on the vestibular mucosa of the right alveolar ridge. The lesion emerged post-extraction, leading to a recommendation to suspend prosthesis use for 4 months and commence topical antifungal treatment. She denies smoking. The biopsy specimen revealed fragments of malignant neoplasia characterized by areas of epithelial proliferation in ridges and keratin production. Additionally, epithelial islands without atypia were observed, with delimitation of the basal layer and intimate contact with the bone matrix. These findings confirmed the diagnosis of Verrucous carcinoma, and the patient was referred for oncologic treatment.
