Resumo: The Paracoccidioidomycosis is a deep systemic fungal infection considered one of the most significant in South America, being the second most common deep mycosis in this region and the most common in Brazil. This is a retrospective observational study that aimed to describe and correlate the clinical and histopathological characteristics of 45 female patients diagnosed with oral paracoccidioidomycosis. The mean age of the patients was 44.18 years (ranged from 20 to 68 years). Among them, 22 patients were smokers, 2 were non-smokers, 2 ex-smokers and information was not available in 19 patients. Regarding clinical characteristics, 53.3% presented multifocal ulcerated lesions, while 46.7% had isolated lesions in sites such as the tongue, alveolar ridge, lips, palate, and others. Systemic findings such as hypertension, HIV, pregnancy, and a history of cancer were noted in some patients. Histopathological analysis using Hematoxylin and Eosin staining revealed multinucleated giant cells, granulomas in 55.5% of cases, microabscesses in 75.5% of cases, and pseudoepitheliomatous hyperplasia in 88.9% of cases. Periodic Acid-Schiff (PAS) staining was employed for fungal quantification. Although paracoccidioidomycosis is unusual in women, it should be considered in differential diagnosis of particularly multifocal ulcerated lesions.
