Resumo: Central Giant Cell Granuloma (CGCG) is a benign lesion of unknown etiology with aggressive behavior and rapid evolution, affecting approximately 7% of tumors in the maxillomandibular region. Clinical treatment is not very effective, whereas surgical treatment is comprised of radical surgical resection with clear margins. This paper reports the trajectory of a 21-year-old male patient with a delayed diagnosis due to failure of radiographic interpretation, where the dentist opted for endodontic treatment, 20 months after the first panoramic radiograph. The patient was referred for specialized treatment, where tomographic imaging and an incisional biopsy were performed with a diagnosis of CGCG, which led to partial resection of the mandible and subsequent rehabilitation with microvascularized flap and osseointegrated implants.
