Resumo: A 72-year-old female patient was referred to our oral medicine clinic for evaluation of bone necrosis following a tooth extraction performed 5 years ago. The patient had been using Alendronate for 5 years. Despite having been evaluated by several professionals, no resolution was achieved. On clinical examination, an area of exposed bone with purulent drainage was observed on the inferior posterior alveolar ridged. Panoramic radiographic and cone beam computed tomography displayed an important bone loss. The patient was initially treated with systemic antibiotic therapy. Afterwards surgical removal of the necrotic bone and affected teeth were performed and complete healing was observed. The patient was followed in regular basis for 6 years without clinical and radiographical signs of recurrence.
