Resumo: A 42-year-old male, diagnosed with squamous cell carcinoma located on the lateral border of the tongue, was referred for treatment of mucositis. When carrying out the clinical and radiographic evaluation, the presence of an infectious focus in the jaw was also identified, compromising the structure of 6 teeth, making it impossible to maintain them in the mouth, with the inevitable indication of multiple extractions. The region in question had been subjected to a dose of 70Gy of radiation and showed signs of osteoradionecrosis. A protocol was established that included antibiotic therapy, photobiomodulation support and antimicrobial photodynamic therapy. The patient was followed for a period of 42 months, during which effective control of the local infection was observed. Oral complications resulting from radiotherapy in the head and neck region are a concern, especially when we consider the risk of surgical trauma that can lead to bone exposure in a hypoxic environment.
